Preliminary evidence suggests that levothyroxine absorption may depend on gastric acidity.

Patients who don't produce enough acid...or are taking omeprazole...seem to need about a 25% higher dose of levothyroxine.

We already know that many antacids can bind levothyroxine and reduce its absorption. Patients can usually get around this by separating the doses by 4 hours.
