Duplicate electronic prescriptions (e-Rxs) can be deadly.

In a recent error, an electronic Rx was sent to the pharmacy for warfarin 1 mg, take one tab three times daily.

The prescriber then realized warfarin comes in 3 mg tabs...and sent over a new e-Rx for warfarin 3 mg, once daily to REPLACE the 1 mg Rx.
