You can limit dosing errors with direct oral anticoagulants (DOACs)...Eliquis, Pradaxa, Savaysa, and Xarelto.

Dosing for these blood thinners can be complex. OVERdosing may increase bleeding risk. But UNDERdosing may decrease protection against blood clots and strokes.

When you get a DOAC Rx, try to find out why the med is being used...and note the indication in the computer. Your pharmacist will need this info to make sure patients are getting the right dose.
