Look Out for Pet Rxs on the Owner's Med History

In one case, Rxs for phenobarbital and zonisamide were on an electronic med list for a patient in the hospital. But the Rxs were for the patient's dog, not the patient.
A similar story occurred when a clinician discovered that the enalapril on a med history was really for the patient's dog.
